- Weeks 0–2 · Protect
Calm the swelling, restore quad activation (the single biggest predictor of long-term outcomes), and protect the graft. BFR training starts here to keep quad and hamstring atrophy minimal.
- Weeks 2–12 · Rebuild
Full range of motion, progressive loading, single-leg control, and the manual therapy that keeps the patellar tendon and scar tissue mobile. Most stalled ACL recoveries trace back to skipped work in this window.

Heavy bilateral and single-leg strength, jumping mechanics, deceleration, and the cutting prep most rehab skips. Strength symmetry target: 90%+ before plyometrics ramp.

Most ACL recoveries get to "feels normal" and stop there. That's where re-tears come from. We carry rehab through heavy single-leg strength, deceleration mechanics, reactive cutting, and on-field volume progression — the phases insurance often runs out before reaching.
We use blood flow restriction training from the first week to fight quad atrophy, and dry needling to quiet the guarding patterns that keep the knee from moving cleanly.

- How long does ACL rehab really take?
- Most patients feel functional within 3–4 months, but a true return to cutting sport averages 9–12 months. The data is clear: returning before 9 months — or before passing objective criteria — roughly doubles re-tear risk. We use milestones, not the calendar, to decide.
- Does graft choice (patellar tendon vs hamstring vs quad) change my rehab?
- Yes. Patellar tendon (BPTB) grafts need extra attention to anterior knee pain and quad recovery early. Hamstring grafts need careful posterior chain loading. Quad tendon grafts need a longer protected quad-strength phase. We adjust loading based on your specific graft and your surgeon's protocol.
- Why is BFR so important after ACL surgery?
- Quad atrophy in the first 6 weeks is the single biggest factor in long-term knee outcomes — and you can't load heavy yet. Blood Flow Restriction training builds strength with 20–30% loads, letting you preserve muscle when traditional lifting isn't safe.
- I had ACL surgery a year ago and my knee still doesn't feel right. Can you help?
- Yes — this is one of the most common reasons athletes find us. Lingering issues usually trace back to incomplete strength work or skipped deceleration/cutting prep. We re-test, identify the gaps, and rebuild what your previous rehab didn't finish.
- Will you coordinate with my surgeon?
- Yes. We work from your surgeon's written protocol and communicate directly with their office on milestones, weight-bearing, and return-to-sport timing — so you're never the messenger between providers.
- Do you take insurance?
- We're out-of-network, cash-based. For ACL rehab specifically, this usually means fewer total visits at higher quality — 55–75 minute one-on-one sessions instead of 15-minute shared visits — which is why a lot of our ACL patients prefer it.
